Abstract

The authors describe the different approaches and surgical techniques reported in the literature on laparoscopic splenectomy. They emphasize the advantages gained from the use of the prototype of an atraumatic instrument they have designed and patented for a direct grasp of the splenic sierosa, with the aim of a better exposure of the organ hilus. The system allows up to shorten the operative time hanging the mobilized spleen. The instrument consists of a cone-shaped, silicon rubber suction cup designed with an anti-slip internal surface. The cup is connected to a support arm with a flexible distal end that can be rotated. Traction is exerted with a commonly available suction system. The two interventions performed with the atraumatic device were completed with laparoscopic technique. The average operating time was 110 min.
