Description

Main Objectives of Emp-H Project: 1) Promoting the adoption of healthy lifestyles with the aim to improve the quality of life of people suffering from a chronic diseases (e.g.: diabetes, breast, colorectal or prostate cancer…) 2) Evaluating the effects of a health promoting model delivered in hospital setting on behavioural risk factors of chronic patients, their caregivers, and hospital staff. 3) creating strong liaisons with the hospital catchment area, useful to provide and maintain a suitable environment for a permanent healthy behavioural changes among patients (within and outside the hospital)
